Long Island University is committed to the health and well being of its students. Student health insurance coverage is a key factor in maintaining good health.

Long Island University encourages all students to maintain health insurance while attending the University.

The University requires that certain categories of students maintain health insurance while attending the University.

The enclosed information explains which Long Island University students are required to maintain health insurance.

Please read further below to determine which type of student you are, then read about the insurance requirements for the type of student you are, and then follow the procedures outlined.

PLEASE READ THE INFORMATION CAREFULLY

IF YOU FALL WITHIN A CERTAIN CATEGORY OF STUDENTS REQUIRED TO MAINTAIN HEALTH INSURANCE, YOU AUTOMATICALLY WILL BE ENROLLED IN, AND CHARGED FOR, LONG ISLAND UNIVERSITY’S STUDENT HEALTH INSURANCE PLAN UNLESS YOU SHOW PROOF OF OTHER COVERAGE BY COMPLYING WITH THE WAIVER PROVISIONS BEFORE THE DEADLINE DATE.

YOU MUST COMPLETE THE WAIVER PROCESS AT THE BEGINNING OF EACH ACADEMIC YEAR DURING WHICH YOU ARE REQUIRED TO MAINTAIN HEALTH INSURANCE.

STUDENT HEALTH INSURANCE
CATEGORIES OF STUDENTS

For health insurance purposes, Long Island University has three categories of students:

COMPULSORY STUDENTS: These are, for example, student athletes, international students, students who live in LIU dormitories, or students enrolled in certain academic programs. Please check the “compulsory student” list to see if you are one of these students.

MANDATORY STUDENTS: These are students who travel to foreign countries as part of their LIU studies or as part of other LIU-sponsored activities. Please check the “mandatory student” list to see if you are one of these students.

VOLUNTARY STUDENTS: These are all other students at LIU, who are not either “Compulsory” or "Mandatory" students.

Compulsory and Mandatory Students will be enrolled automatically, and charged for, LIU’s Student Health Plan. Compulsory Students can waive coverage from LIU if they have other valid coverage, but only by completing the waiver process before the waiver deadline. Mandatory Students do not have an option to waive coverage.

Once enrolled, the policy remains in effect for the full policy period, even if you withdraw from the University, or from classes, or your Compulsory status subsequently changes. The annual policy period begins on 8/15/08 (on 1/1/09 for newly enrolled Spring students, on 5/15/09 for newly enrolled Summer students) and terminates on 8/15/09.

To determine which type of student you are, read the information below:

Compulsory Students are:

  • International Students: Foreign students or scholars engaged in educational activities outside of their home country and who are not permanent residents or pending permanent residents.
  • Domestic Travel Program Participants- Students whose course of study or participation in a LIU program involves sponsored travel within the U.S.
  • Residence Hall Students: Students residing in LIU-owned or leased resident facilities.
  • Intercollegiate Athletes: Students engaged in Intercollegiate Athletics.
  • Clinical Fieldwork Participants: Students in any of the following Professional Programs who are enrolled in fieldwork/training courses. Students in these programs must contact their department advisor or refer to the list of qualifying courses at their campus.

Mandatory Students are:

  • Global College Students: Students enrolled in overseas Global College programs
  • Foreign Travel Program Participants- Students whose course of study or participation in a LIU program involves sponsored travel outside the U.S.

Voluntary Students are:

All other Long Island University students are classified as Voluntary Students.


STUDENT HEALTH INSURANCE
COVERAGE REQUIREMENTS

Once you determine which type of student you are, you need to understand LIU’s Student Health Insurance Requirements. Please read the information below carefully.

COMPULSORY STUDENTS – HEALTH INSURANCE REQUIREMENTS AND WAIVER OPTION.

COMPULSORY STUDENTS are required to maintain valid DOMESTIC health insurance. Coverage must include routine, emergency, non-emergency and hospital care in the New York metropolitan area, and all counties of New York State.

Compulsory Students will automatically be enrolled in, and charged for, the Annual school-sponsored health insurance plan on their bursar account unless you waive coverage. You must waive at the start of each academic year during which you are a Compulsory Student.


TO WAIVE LIU’s STUDENT HEALTH INSURANCE
(COMPULSORY STUDENTS ONLY)

Go Online: Compulsory Students can log on to:

  • Select the "Waive Coverage" button.
  • Provide the required information about current, valid insurance coverage.
  • Print and retain the confirmation receipt that shows that the waiver has been received. In the event of a dispute, only this receipt will be proof of compliance with the waiver process.

Waiver Deadline:
TO WAIVE LIU’s COVERAGE, proof of other valid coverage must be received by:

October 20, 2008 for Fall 2008 Semester

February 23, 2009 for Spring 2009 Semester *

June 1, 2009 for the Summer 2009 Semester *

(*only applies to students newly enrolled in a Compulsory program beginning in Spring or Summer session)

Once you have used LIU’s health insurance plan, you will not be eligible to waive the insurance coverage and you will be responsible for payment of the premium.

REMEMBER - Compulsory Students will automatically be enrolled in, and charged for, LIU’s Annual student health insurance plan unless you waive coverage by completing the waiver procedure before the waiver deadline and retain the confirmation receipt.

FAILURE TO COMPLY WITH THE WAIVER PROCESS WILL RESULT IN YOUR BEING CHARGED FOR LIU’s STUDENT HEALTH INSURANCE PLAN, AND YOU WILL BE OBLIGATED TO PAY THIS FEE.

Insurance ID cards are not issued until after the waiver deadline. To request your ID card prior to the waiver deadline, log on to https://secure.visit-aci.com/enrollment/home/LIU.htm, select the “Enroll in a Policy” button, and follow the Early Enrollment instructions to download/print your ID card and a copy of the coverage brochure.


MANDATORY STUDENTS
HEALTH INSURANCE REQUIREMENTS

MANDATORY STUDENTS are required to maintain valid FOREIGN travel health insurance during any semester involving travel as part of a course of study. Mandatory Students will automatically be enrolled in, and charged per-semester for, LIU-sponsored health insurance plans.

Mandatory Students have no option to waive coverage. The foreign health insurance coverage is limited to treatment of injuries and illnesses sustained while traveling abroad. Mandatory Students may upgrade basic coverage for an extra fee.

VOLUNTARY STUDENTS

Voluntary Students are encouraged, but NOT REQUIRED, to purchase medical insurance. Voluntary Students can purchase health insurance coverage through LIU’s agent, USI Affinity Collegiate Insurance Resources. There are three (3) options to choose from.

  • Download information about the 3 plan options
  • Download and complete the enrollment form for the plan you select.
