Room & Board – Medical Forms – Health Insurance

Students enrolled in the Intensive English Program can stay on campus in a C.W. Post residence hall. A variety of meal plans are available. The cost is approximately $4200 per semester (options are available). Please visit the Residence Life homepage for more details about residence halls and meal plan options.

Medical Forms

All international students must submit a medical form.

If you plan to live on campus in the dormitory, click on Report of Health Evaluation and Medical History. Follow the instructions on this form.
If you plan to live off campus, click on Immunization History. Follow the instructions on this form.

Health Insurance

All F-1 students must have health insurance. If you have health insurance, you must submit proof of insurance and complete a waiver form when you arrive on campus. If you do not have health insurance, you must purchase it through the university. The cost is approximately $1200 per year.
