Immunization Requirements
All students must submit their Immunization History. New York State requires students enrolled in New York State colleges and universities, who were born on or after January 1, 1957, to provide proof of immunity against Measles, Mumps and Rubella. This proof (two Measles, one Mumps and one Rubella vaccination) includes either immunization documentation, serological proof of immunity (copy of the laboratory report), or proof of having had the disease (Measles or Mumps only) from the treating doctor (not acceptable for Rubella). All immunizations must have been given on or after the first birthday, and all must have been live vaccines—measles available beginning in 1968 and Mumps available in 1969. Please obtain a signed immunization history from your physician, previous college or high school. The Immunization History Form and Meningitis Immunization Response Form should be submitted prior to the student’s first day of classes.
