Medical Forms

All international students must submit a medical form.

• If you plan to live on campus in a residence hall, click on Report of Health Evaluation and Medical History. Follow the instructions on this form.
• If you plan to live off campus, click on Immunization History. Follow the instructions on this form.

Health Insurance

All F-1 students must have health insurance. If you have health insurance, you must submit proof of insurance and complete a waiver form when you arrive on campus. If you do not have health insurance, you must purchase it through the university. The cost is approximately $700 per year.
