At the end of the second year, students are to select an individual academic/research advisor for the duration of their tenure in the program. Students are to meet with their potential advisor to establish that the supervisor is able to take on the student as an advisee. This form must be completed for the appointment of an advisor to be official. Students wishing to switch their advisor at the end of the third year may do so by completing this form.
Any student wishing to apply to transfer credits from another university or to waive a selected course must complete this form.

For the Assessment of Children course, students are asked to complete several practice test administrations with children subjects. Parents must sign a consent form to allow their children to sit as testing volunteers.
At the end of each semester students are provided with an opportunity to evaluate their courses on a series of dimensions. This process allows for students to provided anonymous feedback to the faculty of the doctoral program about a course and recommendations for changes in the coursework.
During the second year of the program, students are to choose a concentration of study (Serious and Persistent Mental Illness, Family Violence, or Developmental Disabilities). This form commits the student to take both of the academic courses that pertain to the concentration selected
Students in clinical externships (2nd, 3rd, and 4th year) are to be evaluated on their performance at the externship site. These evaluations are to occur at mid-year and at the end of the externship. Students are to provide their clinical supervisors with the evaluation form.
All doctoral students receive a copy of the Clinical Psychology Doctoral Program handbook, which outlines the scheduled procedures and guidelines of the program. Students are required to read this handbook so that they may be familiar with program policies, and sign this form to attest that they have read and understand the content of the handbook.
All students are required to participate in a HIV workshop. The workshop is conducted at the program and is held every other year. Below please find a copy of the HIV workshop memo, which outlines the workshop requirement, as well as provides a space for students to indicate if they have already taken the workshop at C.W. Post or another program or if the student intends to take the workshop at the next offering.
When you have met the requirements for graduation, fill this form out and send it to the registrar, in order to graduate.
This form must be submitted before you can drive the Clinical Psychology van.
This form must be completed by the dissertation committee at the defense. It must also accompany a student's final draft when they send it to their committee members prior to the proposal and the defense.
This form is completed once a student's dissertation is formally proposed and accepted by the faculty.
Once a student has completed his or her dissertation, it must be bound. Students are permitted to bind their own dissertations, as long as there is a clear plastic cover and a thick black backing. If you would like to use the university binding service, you must use this form and contact the library directly for more information.
